    The 2022 Annual Performance Review process is now underway! The 2022 online Performance Planning and Review system is now open for you to document performance feedback for your direct reports for the year.  To learn more about the Performance Review process, and completing annual year end reviews, please attend one of the following Performance Planning and Development Workshops. All are offered via Zoom.

Reminder, you can write your Annual Performance Reviews in a Word Document and upload into the system.

    03.02.22  IRS Form 1095-C
    Your 2021 IRS form 1095-C (Employer-Provided Health Insurance Offer and Coverage) is now available on eServe!

     
    To Access your 1095-C Form:
    1. Log in to eServe via  access.tufts.edu/eserve 
    2. On the landing page, click on the drop-down menu at the top of the page and select “Employee Self Service.”
    3. Click on Benefits Information.
    4. Click on Affordable Care Act dropdown and Select View Form 1095-C.
    5. Click on 1095-C Original to view/print your 2021 IRS form 1095-C.
    6. Click on the Instructions to view/print the IRS instructions.
    7. If you have not consented to an electronic form, a paper form will be mailed to your home address.  Please allow 7 business days from the postmark deadline of March 2, 2022 for delivery.  The electronic form is also accessible to you on eServe.
    8. Want to opt out of having a paper form mailed to you going forward? Click on Form 1095-C Consent and elect to receive an electronic form only.
    Forms 1099-HC and 1095-C Notes:
    • You do not need to file IRS form 1095-C with your Federal tax return but may use information from the form to complete your return.
    • You should have received form 1099-HC from your health insurance carrier which serves as proof of health insurance coverage for Massachusetts residents. This proof of coverage is required to be filed with your MA state tax return. Form 1099-HC was mailed directly to plan participants from their health insurance carrier by January 31, 2022.
    • If you were enrolled in a health plan through Tufts University and need additional copies of your 2021 form 1099-HC or have questions about the form,  please call Tufts Health Plan Member Services  at 844-516-5790.

    03.01.22  Fitness and Weight Watchers
    Your previous health plan enrollment qualifies you for reimbursement of eligible 2021 Fitness and Weight Watchers expenses.  This is a reminder that you have until March 31, 2022, to submit your eligible 2021 Fitness and Weight Watchers reimbursement requests to Tufts Health Plan (our health plan insurer for 2021).  The 2021 reimbursement forms are available here.  If you have any questions regarding your Fitness and/or Weight Watchers reimbursement, please call a Tufts Health Plan Member Services Specialist at (844) 516-5790.


    Reimbursement guidelines:

    Fitness Reimbursement
    Eligibility:
    • You must have been enrolled in a Tufts University Health Plan for at least 3 months during 2021 to be eligible. 
    • The subscriber plus one other enrolled dependent (age 18 or older) may submit for reimbursement.
    Qualified Expenses: 
    • Up to a 3-month reimbursement per calendar year for a fitness center membership OR up to 24 group exercise classes within a 12-week period.
    • Fitness center memberships qualifications:
      • Subscriber and eligible dependent must be a member of the center for at least 3 months before qualifying for reimbursement.
      • Fitness center must offer cardio and strength training equipment.
    • Group exercise class qualifications:
      • Can take place in a studio or health club setting
      • On-line programs are included (benefit expanded due to COVID-19 restrictions)
      • Qualified classes include aerobics, cycling, yoga, Pilates, Zumba, and kickboxing
    Exclusions:
    • Dependents under age 18 are not eligible.
    • The subscriber or dependent who does not meet the criteria of coverage for the entire period covering the reimbursement request.
    • Fitness center expenses do not include martial arts centers, gymnastic centers, country clubs, aerobics-only or pool-only centers, sports teams and leagues, social clubs and tennis clubs, personal trainers, sports coaches, or the purchase of personal or at-home exercise machines.
    • Group exercise expenses do not include dance classes and classes held in a residential setting.

    Weight Watchers Reimbursement 
    Eligibility:
    • You must have been enrolled in a Tufts University Health Plan for at least 3 months during 2021 to be eligible. 
    • The subscriber plus one other enrolled dependent (age 13 or older) may submit for reimbursement.
    Qualified Expenses: 
    • Up to 12 weeks of Traditional Weight Watchers Meetings OR
    • Up to 12 weeks of Weight Watchers At Work program
    Exclusions:
    • Dependents less than age 13 are not eligible.
    • The subscriber or dependent who does not meet the criteria of coverage for the entire period covering the reimbursement request.
    • Weight Watchers online and Weight Watchers At Home programs are not eligible.
    • In addition, fees for individual nutrition counseling sessions, food, books, videos, scales or other items do not qualify for reimbursement.
